(2) - Software as a medical device (SaMD), downloadable, for predicting analyte levels and adjusting infusion pump settings based on personalized data; downloadable software in the nature of applications for mobile phone and smart phones providing information and education on managing, treating and living with chronic disease; batteries; battery chargers.
(3) - Medical devices, namely, infusion and injection devices for administering drugs and measured amounts of solutions into the bloodstream over time; apparatus for monitoring blood analyte levels; wearable subcutaneous drug delivery devices, sold empty in the nature of injectors; on-body injectors; chronic disease management devices, in the nature of medical sensors that are implanted or inserted into the human body used to continuously monitor the concentration of analytes in the human body and accessories therefor, namely, receivers, sensor housings and sensor insertion devices; medical infusion sets comprised of infusion tubing, infusion needles, needle guards, connectors, attachment clips, and accessories therefor; pouches and mounts for use with infusion pumps.
(4) - Administration of a trade-in and upgrade program for drug delivery and chronic disease management devices; mail order, online and telephone pharmacy retail services featuring chronic disease management supplies medications, drugs and other prescriptions.
(5) - Educational services, namely, creating and developing web-based and in person classes, seminars, workshops and programs in the field of managing and living with chronic disease; personalized training and coaching in the use of monitoring devices and infusion pumps in the treatment of chronic disease; training of healthcare providers in chronic disease treatment and the use of infusion pumps and infusion sets.
(6) - Software-as-a-service (SaaS) featuring software for use in the acquisition, capture, processing, presentation, transmission and storage of medical and physiological data obtained from medical sensors that are implanted or inserted into the human body; hosting an interactive web site featuring technology that enables users to enter, access, track, monitor and generate health and medical information and reports relating to managing chronic disease, medication and analyte levels.
(7) - Interactive medical monitoring, reporting and alert services for infusion pump users for monitoring and managing chronic disease; providing medical information related to chronic diseases, including advice on treatment options and chronic disease management from a web site; providing medical information for consumers and healthcare practitioners via online databases; medical consultation services, namely, patient ambassador program to communicate medical information to people with chronic disease.
